There are multiple providers, sharing space and staff.
Paying for visits is appreciated at time of business. For those that have insurance we bill your insurance through Spectrum Billing. It is the patients’ responsibility to know their copays and deductibles. The front office is happy to take payment but they do not know your personal insurance issues.
Debbie at Spectrum Billing (907) 440-5900 will take care of your billing needs. They can preauthorize you, so please call them with your insurance information. We will be billing secondary insurance as well when needed.
The first visit typically lasts 60-90 minutes in order to get a detailed history, perform appropriate physical exams, order any labs needed, and give a detailed treatment plan. In my practice, I prefer to utilize multiple treatment modalities to effectively and creatively address the causes of your particular health concerns. Return visits typically last 30 minutes. Any acute visit for an established patient will typically last about 20-30 minutes.
Prior to your appointment there are forms that need to be filled out, the bulk of those are here, if you don’t fill these out prior to the appointment please come 15 minutes early to complete them.

Please fill this out if you are new to the practice or have not seen me in 2 years.
b New Patient History-Pediatric
Please fill out for all patients prior to puberty or 15 years old.

All patients need one in the chart.
I use the email for patient reminders so please include it in your demographics. It also includes your insurance information if you have insurance. We will take a copy of the card as well as ID if possible, at first visit.

For new patients I often have you release records from previous health care providers, particularly labs in the past 5 years.
For previous patients I will need a release for any new visits to your other doctors and older records, particularly labs. Also if you are returning after an absence of longer than 3 years please fill out all the forms.
If you have previous labs please bring them to your appointment. I encourage patients to keep a file of their labs in order to keep a record of their own health history.
